Your named GP will have overall responsibility for the co-ordination of all appropriate services, care and support that our surgery provides you. It does not prevent you from seeing any GP in the practice as you currently do.
Welcome to the Family Surgery. Please select the service you require from our list of services and clinics including minor skin surgery. We are pleased to welcome three new partners Dr Rob Caudwell, Dr Bapi Biswas, and Dr Rosalind Bonsor. We are pleased to Welcome Dr Wendy Coulter who has joined us as a GP Principal along with our Nurse Clinician Mary Burns, Dr Jenny Rigby and Practice Nurse Maria Jordan to the regular clinical team.
You have told us that sometimes you find it frustrating and difficult to get the appointment you need, when you need it. By offering telephone appointments and giving advice, we can ensure that everyone is able to speak to, or see, a Clinician at a time and day they choose.
Many people don't need to come into the surgery to be seen at all; their problem can be dealt with by telephone. This gives your doctor more time to see those people that need appointments in the surgery. As your doctor is the one making the appointments, he or she can judge how much time you might need and give you a long or short appointment. This helps appointments to run to time and means you're more likely to be seen on time. We also know that when people make an appointment close to the date that they call, they are more likely to keep the appointment.
Pre-bookable appointments are available upon request. Appointments may be arranged via telephone between 08:00 & 09:00 each working day.
One issue per appointment per patient.
Urgent requests are assessed by triage.
The Nurse and Health Visitor can be pre-booked up to 4 weeks in advance.
Please advise the surgery if you are unable to keep your appointment.
Home visits may be arranged in the case of End of Life patients, or those who are completely housebound. We are unable to take into account problems with transport or other non medical reasons for home visit requests.
The surgery has a single disabled car parking space to the rear of the building and a ramp to allow ease of access to the surgery.
We have a practice based Pharmacist who attends on a weekly basis to undertake medication reviews. You will be advised to attend where this is indicated.
Please request prescriptions in writing or online, we do not take prescription requests over the phone.
Note: The surgery requires 48 hours to process prescription requests.
Please allow two working days for collection from the point of ordering.
Please book your Flu Jabs via reception. Children between 2-3 years old are also being offered the vaccination.
You are now able to make appointments and request medication on line.
You can either register on line through this link to the Patient Access website and await acceptance from the Surgery, or call into the Surgery were you will be given a personal PIN number to use in the registration process on line"
SHARING PATIENT DATA if you wish to opt out of sharing your patient data with other NHS services then please contact the practice asap.
7-Day GP Service
If you are a registered patient at The Family Surgery you can now book non-urgent evening and weekend appointments with a GP, nurse or other healthcare professional through a new service launched in October 2018.
Local practices are working together via our GP Federation, Southport & Formby Health Ltd, to offer patients better access to evening and weekend appointments in Southport & Formby through a new service called 7-Day GP Service.

We hope that this new service will help us to deal with patient demand at busy times and help working-age patients access appointments at the evening and weekends.
